Nursing Home Quality
Better Together.Specifically, we are partnering with nursing homes to:
- Improve the Five Star Quality Domain Score Rating that includes 15 quality measures
- Reduce adverse drug events with a focus on anticoagulants, diabetic agents, opioids and antipsychotic medications
- Increase resident safety
- Reduce healthcare acquired infections and conditions
- Improve appropriate and quality care transitions with an emphasis on the reduction of emergency department visits and readmissions
We will meet nursing homes where they are at in their quality improvement journey. Participating homes have access to expertise and technical assistance from our team. Best practices, data, tools and trainings are available to support nursing homes in achieving set quality improvement aims. We identify and facilitate educational opportunities and peer networking while utilizing data as a driver for change.
Thursday’s Boost
The Great Plains QIN team will provide updates, guidance, resources and education on topics pertinent for nursing homes, including infection prevention, NHSN reporting, CMS-initiatives and partner updates. An office hours format will follow for you to ask questions of our team and your nursing home peers. Each Boost With Your QIO will be 30 minutes, and you only need to register one time.
