Nursing Home Quality

Better Together.

We look forward to partnering with and supporting nursing homes across our region over the next five years. We will work together to implement quality improvement strategies to achieve overall better care, lowered healthcare costs and better health outcomes.

Specifically, we will partner with nursing homes to:

  • Improve the Five Star Quality Domain Score Rating that includes 15 quality measures
  • Reduce adverse drug events with a focus on anticoagulants, diabetic agents, opioids and antipsychotic medications
  • Increase resident safety
  • Reduce healthcare acquired infections and conditions
  • Improve appropriate and quality care transitions with an emphasis on the reduction of emergency department visits and readmissions

We will meet nursing homes where they are at in their quality improvement journey. Participating homes will have access to expertise and technical assistance from our team.  Best practices, data, tools and trainings will be available to support nursing homes in achieving set quality improvement aims. We will identify and facilitate educational opportunities and peer networking while utilizing data as a driver for change.
