Join Our Efforts to Improve Care and Health Outcomes

If you are a nursing home provider in our region, we invite you to partner with us. The focus of our efforts will be enhancing consumer engagement, strengthening survey processes and standards and collaboration across multiple long-term care providers in all eligible nursing homes.

Nursing homes throughout the region are partnering with Great Plains QIN to improve processes of care and improve Healthcare-Acquired Conditions (HACs). Specific efforts for improvement include pressure ulcer reduction, elimination restraints, a decrease in antipsychotic medication use and a decrease in resident falls. In addition, nursing homes will work to improve processes to ensure safer and more person-centered care.

Great Plains QIN invites all nursing homes to join our fast-paced, dynamic, data-driven quality improvement initiative to address system-wide issues and opportunities for improvement. Utilizing the Quality Assurance Performance Improvement (QAPI) training, tools and resources, we hope to prepare nursing homes for the upcoming changes in the regulations and provide a solid foundation for improvement efforts.

Learning and Action Network

We invite you to join the Great Plains Learning and Action Network (LAN). All LAN partners will be invited to attend educational sessions on a variety of topics, have opportunities to learn from peers throughout the state and region and have access to an abundance of resources and tools. The LAN is a great opportunity to get connected and demonstrate your commitment to quality improvement.
