Electronic Outpatient Antibiotic Stewardship Toolkits
Greetings Leaders in Outpatient Antibiotic Stewardship!
The purpose of this toolkit is to provide outpatient antibiotic stewardship leaders with the resources needed to implement a program inclusive of The Core Elements of Outpatient Antibiotic Stewardship as defined by the Centers for Disease Control and Prevention. It will serve as an aid for tracking your team’s development and implementation progress. Additional resources and updates will be shared in an electronic version of this toolkit which will be made available soon.
Commitment is a determinant of success. Activities and interventions that will fulfill commitment are noted on the checklist adhered to the content organizer for this section. By completing the Participation Agreement and returning it to QHA with a signature, you have identified a leader to direct antibiotic stewardship activities in your facility. Congratulations!
In the content organizer you will find posters to display public commitment.
In addition to commitment, policies and interventions will drive appropriate antibiotic utilization practices. The checklist adhered to the content organizer for this section provides suggested strategies and examples are provided in the content organizer. Prioritize chosen interventions according to feasibility, acceptability, resource commitment and anticipated barriers.
Also included in this section is an Outpatient Antibiotic Stewardship Plan template. This template is designed to guide development and implementation of your facility’s antibiotic stewardship program. The content captured in this plan may be useful as you develop and/or refine your facility’s outpatient antibiotic stewardship policy. A community pharmacy specific plan will be provided shortly.

Tracking and reporting is necessary to focus interventions and gauge improvement. Start small and build your tracking and reporting system as you move forward. The checklist adhered to the content organizer in this section offers several suggestions to guide tracking and reporting implementation; a sample tracking tool is provided in the content organizer. Also, an electronic Infection Tracker, developed by QHA and the ND Department of Health, will be made available soon.
Remember to set up a formal process for reporting to your facility’s executive leadership, committees and staff.
