The South Dakota Department of Health has formed the Community Information Exchange (CIE) and is seeking interested partners to assist with long-term planning.
The Community Information Exchange (CIE) is a statewide collaboration of health care, human and social service providers sharing information using an integrated technology platform and referral system to coordinate whole-person care. The vision is to streamline connection between the entities to address social needs and advance health improvement among populations at higher risk and that are underserved.
The Department wants to ensure the CIE meets the needs of as many partners as possible, across various sectors throughout the state. Review the Partner Letter for more information on the CIE, the timeline and an opportunity to explore a future interface of the CIE with your organization’s preferred technology platform(s).
Additionally, the CIE Announcement Handout lists some of the potential benefits at the individual, partner and community levels. It also provides a link to where you can find more information through Helpline Center partners.
If your organization is interested in interfacing with the CIE through your electronic medical record or other supported technology, please complete the CIE Partner Request for Interface. The Department will share updates as the CIE implementation progresses.
