Join Our Efforts to Improve Care and Health Outcomes
If you represent a Home Health Agency (HHA), we would like to invite you to partner with us.
The Great Plains QIN is assisting HHAs improve cardiac care for their patients by providing training and technical assistance on successful interventions, tools, and Health Information Technology (HIT). Cardiac care of all patients will be addressed with a special focus on those with the greatest need. We are also facilitating opportunities for HHAs to improve coordination of care by collaborating with partners in their local communities.

Special Funding: Infection Prevention
We were recently awarded special funding to work with home health agencies in our region to develop and test educational interventions to prevent and manage common infections in home health patients; such as respiratory, urinary tract and wounds.
A significant number of patients admitted to home health develop infections; estimates are as high as 1.2 million per year, with the predominant risk factor being the presence of a medical device (i.e., catheter). Additionally, 20 percent of home health patients require wound management. The reality is that few home health specific infection prevention and management tools and resources exist. With the expanded scope and intensity of home health services, infection prevention and control are more critical than ever.
This innovative work is a unique opportunity being piloted in our region. Participating agencies will work in tandem with Great Plains QIN to decrease home health acquired infections (respiratory, urinary tract and wounds) through improved access to infection prevention education, resources and information.
If successful, these infection prevention and management resources will be spread nationally via the Home Health Quality Improvement (HHQI) National Campaign.
Learning and Action Network
We invite you to join the Great Plains Learning and Action Network (LAN). All LAN partners will be invited to attend educational sessions on a variety of topics, have opportunities to learn from peers throughout the state and region and have access to an abundance of resources and tools. The LAN is a great opportunity to get connected and demonstrate your commitment to quality improvement.
