The Participation Agreement below outlines benefits and expectations.

Great Plains Quality Care Coalition Participation Agreement

Our organization agrees to participate in Great Plains Quality Innovation Network (GPQIN) supported coalitions. We understand this commitment requires support of our leadership and we agree to:

 

  • Commit to remain active in the GPQIN supported coalitions through 2024
  • Be publicly identified as a coalition participant and share best practices and lessons learned
  • Collect and share available data to measure the impact of interventions to quality improvement outcomes
  • Participate in educational sessions, teleconference calls and/or webinars on specific clinical topic areas, systems improvement and leadership
  • Apply quality improvement strategies and principles to support the CMS five broad aims to:
    • Improve Behavioral Health Outcomes focusing on Decreased Opioid Misuse
    • Increase Patient/Resident Safety
    • Chronic Disease Self-Management (Cardiac & Vascular Health, Diabetes, Slowing & Preventing ESRD)
    • Increase Quality of Care Transitions
    • Improve Nursing Home Quality
    • Other topics that may become relevant during the stated timeframe

Great Plains QIN Agrees To: Participating Organizations Agree To:
Provide expertise, practical and technical assistance, best practices, data, tools and resources and trainings to support quality improvement aims through 2024. Commit to remain active in the coalition through 2024. Agree to be publicly identified as a participant in the coalition and share best practices and lessons learned with coalition participants.
Develop and facilitate educational opportunities around CMS broad aims: Improve Behavioral Health Outcomes focusing on Decreased Opioid Misuse; Increase Patient Safety; Chronic Disease Self-Management (Cardiac & Vascular Health, Diabetes, Slowing & Preventing ESRD); Increase Quality of Care Transitions; Improve Nursing Home Quality; and other topics that may become relevant during the stated timeframe. Participate in educational sessions, teleconference calls and/or webinars on specific clinical topic areas, systems improvement and leadership.
Facilitate and support community coalitions to form a sustainable structure. Connect nursing homes to community coalitions. Maintain engagement in the coalition by participating in meetings and contributing to community interventions. Communicate organizational contact changes with GPQIN.
Utilize available data sources to measure change impact on CMS broad aims [noted above]. When necessary, data requests may be made to the organization for data that cannot be obtained from available sources. Collect and share data as needed to measure the impact of community and organization interventions on outcomes.
Align GPQIN supported community coalition efforts with state and national initiatives and partner with stakeholders to minimize duplication. Apply quality improvement strategies and principles to CMS broad aims: Improve Behavioral Health Outcomes focusing on Decreased Opioid Misuse; Increase Patient Safety; Chronic Disease Self-Management (Cardiac & Vascular Health, Diabetes, Slowing & Preventing ESRD); Increase Quality of Care Transitions; Improve Nursing Home Quality; and other topics that may become relevant during the stated timeframe.
