Great Plains QIN Huddle: Minimum Cognitive Impairment

The Great Plains Quality Innovation Network team will host a ‘Huddle’ after each Project TeleEcho – Geriatric and Age Friendly Care session hosted by the University of North Dakota. These huddles will further the discussion on how each topic relates to the 4Ms framework and how to incorporate into your area of practice. We will answer questions and offer assistance and resources. The Great Plains QIN Huddles are scheduled the third Tuesday of the month from 12:00 – 12:45 p.m.
The University of North Dakota is hosting a Project TeleEcho – Geriatric and Age Friendly Care; utilizing the Age Friendly Health Systems 4Ms Model of Care. The Geriatric and Age-Friendly TeleECHO offers:
- Free educational credits for healthcare professionals
- Collaboration, support and ongoing learning with subject-matter experts and healthcare staff
- New ways to support patients in their home community

All sessions will be recorded. Healthcare providers in North Dakota and South Dakota are invited and encouraged to attend.
