Bacteria can become resistant to antibiotics and continue to multiply in the body, making life-threatening illnesses more difficult, or even impossible, to treat. Staying up-to-date with immunizations to prevent illness and only taking antibiotics when needed are ways that we can protect our health. Great Plains QIN developed the Connecting the Dots tools and resources to increase knowledge and achieve better health outcomes through understanding the connection between sepsis, immunizations and antibiotics. Krystal Hays serves as the Regional Project Manager for the Great Plains Quality Innovation Network and is passionate about meeting healthcare providers where they are in their quality improvement journey and working side by side to create healthcare systems that provide Medicare beneficiaries with better care, better health, exceptional service, at a lower cost.
Krystal is a registered Nurse with over 30 years of healthcare experience beginning as a home health aide. She has held staff and leadership positions in rural nursing homes and critical access hospitals, which has provided insight into the unique challenges experienced by these organizations in serving the healthcare needs of diverse rural populations. Krystal received her Doctor in Nursing Practice from the University of Nebraska Medical Center and is a Certified Resident Assessment Coordinator, TeamSTEPPS® Master Trainer, Certified Dementia Practitioner, and Certified Alzheimer’s Disease & Dementia Care Trainer.
Tammy Wagner is a Quality Improvement Advisor with Great Plains Quality Innovation Network-Quality Improvement Organization who partners with hospitals, nursing homes, home health, and public agencies to improve care within our communities and across care settings.
With over 30 years working in multiple healthcare organization settings and multiple roles from CNA to leadership. Tammy understands the diverse populations and the need for these populations to have access to care that is safe, accessible, and affordable, improving communication and transitions in care for the patient and family between healthcare settings.
Tammy is a Registered Nurse, TeamSTEPPS® Master Trainer, Lean Six Sigma Great Belt, Certified Dementia Practitioner, Certified Alzheimer’s Disease Certified Trainer and has her certificate in Antimicrobial Stewardship Certificate of Mastery.
