Attend this WebEx to learn more about improving care for people with multiple chronic conditions that are often complicated by patients’ limited ability to care for themselves independently and by their complex social needs. Improving care coordination for these ‘super-utilizers’ will help improve the performance of the health system.
Clinics, hospitals, pharmacies, home health, hospice, public health, consumers and families and all community partners who will benefit from a discussion addressing super-utilizer patients are encouraged to attend.
Key strategies to help health systems and communities implement complex care teams as well as identify key data and metrics for program measurement will be addressed. The role of technology, non-traditional workforce and patient-stated goals in complex care management will also be shared.
