Diabetes Alert Day is March 28, 2017

The American Diabetes Association Alert Day serves as a “wake-up call” to inform the American public about the seriousness of diabetes.The goal is to encourage individuals to join the movement to Stop Diabetes® by taking the Diabetes Risk Test and finding out if they, or their loved ones, are at risk for developing type 2 diabetes.

We would like to encourage all healthcare providers, providers, stakeholders and staff to help raise awareness about the risks of diabetes and the availability and value of diabetes self-management education classes offered in your community. We also ask you to encourage your patients and loved one to take the Diabetes Risk Test online, which asks a few simple questions about weight, age, family history and other potential risk factors for pre-diabetes or type 2 diabetes.

The Great Plains Quality Innovation Network is working to improve health outcomes and reduce issues of health disparities among people with diabetes through the following:

  • Assisting physician clinics in improving utilization and clinical outcome measures for HbA1c, lipid, blood pressure and weight control for Medicare consumers with diabetes
  • Improving HbA1c, lipid, blood pressure and weight control outcomes for Medicare consumers with diabetes
  • Decreasing the number of individuals who require lower extremity amputations due to complications resulting from poorly controlled diabetes
  • Increasing Medicare consumer participation in Diabetes Self-Management Education (DSME) classes
  • Increasing the number of diabetes educators, Certified Diabetic Educators (CDEs) and community health workers
