Mirriam Webster’s defines chronic as continuing or occurring again and again for a long time. While that certainly “applies” to chronic conditions, such as high blood pressure, hyperlipidemia and Type 2 Diabetes – there are real opportunities to bring those diseases into control.

We are eager to build on the demonstrated impact of programs, like Target: BP. Last year more than 1800 organizations, many of them rural-serving, earned recognition and engaged in various levels of quality improvement. Ensure your organization – as a whole and/or by site – will join (or remain) on that list in 2024!

Here’s what to do next:

  1. Visit our Outpace CVD homepage to begin accessing our suite of resources for all three ambulatory quality efforts.
  2. Register for access  to the data submission  portal for the selected  program(s).
  3. Consider joining the overview webinar scheduled for 1 pm on January 24th.
  4. Apply for our rural health grant opportunity, open until 1/15/24.
