Mirriam Webster’s defines chronic as continuing or occurring again and again for a long time. While that certainly “applies” to chronic conditions, such as high blood pressure, hyperlipidemia and Type 2 Diabetes – there are real opportunities to bring those diseases into control.
We are eager to build on the demonstrated impact of programs, like Target: BP. Last year more than 1800 organizations, many of them rural-serving, earned recognition and engaged in various levels of quality improvement. Ensure your organization – as a whole and/or by site – will join (or remain) on that list in 2024!

- Register for access to the data submission portal for the selected program(s).
- Consider joining the overview webinar scheduled for 1 pm on January 24th.
- Apply for our rural health grant opportunity, open until 1/15/24.
