Nursing homes across the four-state region are advancing quality improvement efforts and achieving quality composite score success!  Great Plains QIN distributes a quarterly composite score report with easy-to-read graphs, trending and comparisons recognized nursing home with a score of 6.0 or less.  The composite score is derived from 13 quality measures and is used to monitor progress at a facility, state, regional and national level for the National Nursing Home Quality Care Collaborative.

Various tools and resources, including the Quality Milestones Recognition Program and the Quality Measure Video Series have proven to be instrumental for continual progress.

Quality Milestones Recognition Program

While optional, the “Milestones – QAPI in Action” project was designed to help track quality improvement efforts and achievements . The Milestones booklet and templates are available in an electronic version providing a format for easily tracking and recording progress. Each milestone has a checklist and link to report progress and recognize achievement levels.

Quality Measures Video Series

Healthcare professionals interested in learning more about the quality measures and formula for the composite score can access a series of short quality measure videos (10-20 minutes).  Information on quality measure numerator, denominator and exclusions along with MDS tips are available as on-demand education.  Each quality measure has a handout including a printable video transcript for easy reference.
