Despite best efforts of healthcare providers/professionals, approximately 1 in 10 patients is harmed by the care they receive. Effective communication following harm can leave patients and families wondering what happened and seeking legal action to find answers. The Agency for Healthcare Research and Quality (AHRQ) released a new online toolkit, CANDOR, to help hospital and health system leaders and clinicians communicate with patients and their families when something goes wrong with their care. The CANDOR process is one which healthcare institutions and practitioners can use to respond in a timely, thorough and just ways when unexpected events cause patient harm.
