In partnership with Service Providers for Seniors, Dakota Geriatrics is excited to announce the third installment of the Age Friendly Health Care: Webinar Series.

4Ms Model of Care ImageThis version will focus on Chronic Disease Management and include 9 one-hour tele-mentoring sessions to be held every 3rd Monday of the month at 4 pm (CT), starting October 17, 2022. These will be facilitated by Dr. Donald Jurivich, Professor and Chair, Department of Geriatrics, University of North Dakota. All health care providers are welcome to attend.

Continuing Education credits will be provided for attendees following live events, and submission of mandatory evaluation form as approved by North Dakota Board of Nursing, North Dakota Board of Social Worker Examiners, North Dakota State Board of Occupational Therapist Practice and North Dakota Board of Physical Therapy.

*Registration is required for obtaining continuing education credits.
