One mission of CMS is to ensure that patients admitted to the hospital receive proper care from their physicians as well as an effective discharge plan. The idea is that if patients leave with a future-care plan in place, they will not walk back through hospital doors days or weeks later.
Improving care coordination leads to better patient outcomes, overall satisfaction and reduces avoidable hospital admissions. Great Plains QIN will partner with communities throughout the region to unite stakeholders, consumers and healthcare providers to improve communication and care coordination – resulting in reduced hospital admissions, readmissions and medication harm.
