On August 10, 2017, QHA will be hosting their annual Quality Forum at the Delta by Marriott in Fargo, ND, under the theme “The Power of Engagement…Linking Patients, Providers, Communities and Payers.” This one-day event will focus on how engagement of individuals at every stage in the healthcare continuum contributes to the desired effect of better clinical outcomes, reduced costs and improved care coordination. The goal of the Forum is to provide attendees with a solid understanding of engagement (inclusion) as well as real life implementation examples.

A family member will share the difficult journey through her mother’s lengthy illness highlighting challenges with medication reconciliation and discharge summaries.
Jay D. Bhatt, DO, MPH, MA, FACP, with the Health Research & Educational Trust/American Hospital Association, will set the stage with “A Framework for Engagement” where he will discuss the levels of engagement (inclusion of voice) across the healthcare continuum and how this multidimensional framework can be used for the development of interventions that ultimately improve patient outcomes for healthier communities. Dr. Bhatt will also facilitate the closing session that will help attendees synergize their learnings and set action goals.
North Dakota stakeholder organization representatives will share their perspectives on engagement as it relates to consumers, physicians, hospital administrators, rural hospitals, long-term care frontline caregivers and residents, pharmacists and payers.
Breakout sessions will include the topics: antimicrobial stewardship, advanced care planning, quality payment program, teach-back, opioids, colorectal cancer screening, patient-centered medical home and quality assurance/performance improvement planning.
