What is the most important conversation that we should be having, but avoid at all cost? It is the conversation about how we want to live until the last day of our life, part of the advance care planning process. In this COVID pandemic, more people are facing their mortality and recognizing the value of these most critical conversations. Have you had the conversation?

Objectives
• Identify three measures to bring awareness of Advance Care Planning (ACP) to your facility.
• Describe two action steps to educate colleagues about ACP conversations.
• Differentiate two distinctions between a healthcare directive and a POLST/MOST form.

Nancy Joyner, RN, MS, APRN-CNS, ACHPN®

Nancy Joyner is a nationally recognized consultant, speaker, educator and author. She is a self-employed Palliative Care Clinical Nurse Specialist. Nancy has gained extensive experience in serious illness, frailty, and end of life over 40 years of nursing, with 15 years as an advanced practice nurse in palliative care across the lifespan from neonatal care to the elderly. She has been the subject matter expert for palliative care for UND’s Center for Rural Health’s Rural Community-Based Palliative Care project. Nancy is an approved educator for the Hospice and Palliative Nurses Association and a certified First Steps ACP Facilitator Instructor. She is a nationally trained POLST trainer who has written and published on the topic of advance care planning & POLST. Nancy is currently president of Honoring Choices® North Dakota and North Dakota’s POLST Program Coordinator.
