“As I reflect on my years as a pharmacist, I find myself compelled to promote cautious (prudent, or judicious) use of medications. I remember my days as a pharmacy student and my excitement that drugs were THE answer in the treatment of illnesses and diseases. You name it, there was a pill for it, and if not, we needed one. While this is in part is true, we must recognize that there is inherent risk with the use of medication,” stated Sorenson.

She continued, “After publications in the 90’s on the incidence of adverse events, the spotlight on patient safety became brighter. Today, we find ourselves continuing to face this highly significant patient safety issue. One of the most difficult things I observed while working in direct patient care, was patient harm from adverse effects and medical errors. We must be intentional about validating the need for every medication for every patient at every step of the medication use continuum.”
