Knowledge Is Power

The Great Plains QIN team had the opportunity to partner and support the University of North Dakota Project ECHO Series; the focus being Geriatric and Age Friendly Care. Following each monthly ECHO session, the Great Plains QIN team offered a ‘huddle’ to further the discussion on how each topic relates to the 4Ms framework and how to incorporate into practice. Personal stories were shared and resources offered.

Partnering with the University of North Dakota on this TeleECHO series was an opportunity to collaborate, support and learn from subject matter experts. Session leaders helped identify knowledge gaps and fostered understanding of effective, high quality medical and behavioral treatments for conditions common among the senior patient population. We worked to identify ways to support individuals in their homes and community.
