Using EHR to Establish a Workflow Presentation

The toll of having a chronic disease such as diabetes can greatly compromise a person’s quality of life.  Evidence-based, self-management education programs have been proven to significantly help people with chronic diseases through teaching participants positive life skills to enhance well-being.  A provider’s recommendation is key in helping patients with chronic diseases enroll in these vital programs.

This webinar showcased how a Federally Quality Health Center (FQHC) utilized Electronic Health Records (EHRs) as they implemented a referral process for self-management programs.  You will hear steps to translating these successes into your practice.

Objectives:

This webinar will enable the participant to:
1. Describe a team-based referral process aligned with EHR system.
2. Define implementation of alert systems/provider reminders for referral to DSME/CDSME programs.
