Overview
During this WebEx, the presenters shared the unique partnership Park Nicollet Methodist Hospital has created with their local firefighters to conduct home visits on high risk patients that would otherwise fall through the cracks. Learn how the partnership was developed, what happens on the home visits and how this innovative intervention has helped to reduce 30-day readmissions.

Objectives:
Upon conclusion of this webex, participants will be able to:
•    Describe an innovative approach in preventing readmissions using firefighters to conduct post-discharge firefighter visits
•    Discuss data and outcomes from the hospital/firefighter partnership
•    Identify how the firefighter visit addresses the triple aim and enhances patient and community resilience
