Overview:

Cindy Sun will present information regarding the Home Health Quality Improvement Campaign (HHQI) CardioLAN and cardiovascular registry, evidence-based tools and a wealth of ongoing educational opportunities. Whether you are a home health practitioner directly providing patient care or an allied partner with a stake in improving the quality of care that home health patients receive, you will not want to miss this informative presentation.

Objectives:

Upon completion of this presentation, attendees will be able to:

  1. Describe the relationship between providing quality cardiovascular care and benefit to the patient and home health agency
  2. Access and incorporate three free evidence-based turn-key tools and/or resources into current practice
  3. List three ways Great Plains QIN can assist home health agencies incorporate data-driven quality improvement into Quality Assurance Performance Improvement (QAPI) in preparation for Conditions of Participation (CoP)
