Coordination of Care Improving Cardiac Care Patient-Centered Care

Heart failure is a top diagnoses linked to readmissions throughout the Great Plains and Lake Superior Quality Innovation Networks. This webinar is the fifth in a series of webinars to assist communities in this seven-state region improve heart failure care.

Target Audience:

Healthcare professionals interested in improving heart failure care including, but not limited to: nurses, heart failure program coordinators, cardiologists, program medical directors, and quality improvement personnel

Objectives:

Upon completion of the WebEx, participants will be able to:

  1. Discuss the importance of engaging heart failure patients in their care
  2. Describe techniques to actively engage heart failure patients in the medical care plan
  3. Hear directly from a patient/family on how they are able to manage heart failure symptoms as a result of actively participating in their care
