Division Chief Paul Miller will share how a Community Paramedicine Program in Crawfordsville, Indiana is partnering with community organizations to navigate people to the right care, at the right time, and right location.
This program is designed to extend services upon hospital discharge, help monitor chronic conditions, assess fall risks in the home, and other services to ensure patient safety and health.
