The Agency for Healthcare Research and Quality (AHRQ) sponsored the development of the Nursing Home Survey on Patient Safety Culture; designed specifically for nursing home providers seeking opinions about the culture of patient safety and healthcare quality in their nursing home. It is not designed for use in assisted living facilities, community care facilities or independent living facilities.

AHRQ LogoVoluntary free data submissions to the Surveys on Patient Safety Culture™ Nursing Home Database will be open September 1 – 21 for nursing homes that have administered the AHRQ Nursing Home Survey on Patient Safety Culture. Participating nursing homes will receive feedback reports displaying their results as well as aggregated, de-identified results from all database participants for patient safety improvement purposes.
