The Centers for Medicare & Medicaid Services (CMS) is hosting a National Care Transitions Awareness Day Summit on April 16, 2019 to increase awareness and action in the beneficiary and provider communities of the importance of safe and effective care transitions between all settings. The format provides an opportunity for CMS to actively listen to individuals, caregivers, healthcare systems, community partners, payers and business entities working to improve care transitions.
The NCTA Day will feature leaders and innovators in health care as well as engaging and interactive speaker sessions throughout the day. The NCTA Day will offer activities that expand awareness and action about the role of community partners, with a focus on providers, payers and other business entities.
The first annual NCTA Day will be held at the CMS Headquarters in Baltimore, MD, for those wishing to attend in person – 9:00 a.m. – 4:30 p.m. [ET]. There is also a virtual participation option available. The deadline to register is April 1, 2019.
