Announcing a New Deadline For Innovative Practice Applications
Due to the high level of interest in the Strategic Innovation Engine’s (SIE) first Call for Innovative Practices, the deadline for applications has been extended to July 22, 2016. CMS is seeking new and adapted practices to improve quality in five topics:
• Managing Chronic Conditions
• Engaging Patients and Families as Active Members of their Care Team
• Coordinating Care Within and Across Settings
• Streamlining Patient Flow
• Integrating Behavioral Health into Care
