SDSMA Center for Physician Resources logo, Provider Perspectives on Patient-Centered Care a partnership webinar series

Overview and Purpose:

Key patient-centered care topics will be presented in 15-20 minute webinar segments highlighting provider and clinician perspectives while introducing corresponding resources and tools during the Provider Perspectives on Patient-Centered Care webinar series.

“Analyzing Patient Panels and Health Outcomes” is the tenth and final webinar in the series. Providers’ patient panels vary by demographics, health concerns and number.  Applying population health concepts to a patient panel can highlight the benefits of engaging in relevant quality improvement efforts.

Joy Gebhard is the current Director of Quality and Accreditation for Avera Health Plans and DAKOTACARE. As Director of Quality and Accreditation, Joy’s primary areas of focus includes oversight of National Committee of Quality Assurance (NCQA) Accreditation,  Healthcare Effectiveness Data and Information Sets (HEDIS), Appeals & Grievances program, Consumer Assessment Healthcare Providers and Systems(CAHPS), and Affordable Care Act (ACA) Risk Adjustment program.

Upon completion of this program, participants will be able to:

1. Compare patient panels to clinic populations
2. Identify priority quality improvement for positive patient outcomes
